Clinic Case Manager
San Diego Youth Services seeks a Clinic Case Manager in the ICARE Program to be based at the San Diego office location. This is a full-time & non-exempt position reporting to the ICARE Program Manager.

- Help youth develop life skills. In addition to a range of safe and stable housing options, we offer support to help youth develop the independent living skills necessary to become self-sufficient and keep off the streets.
- Promote overall health and well-being. Early intervention and care for the whole child are critical. We provide a broad continuum of services to help youth overcome trauma and challenges that put them at risk, including case management, counseling, mental health treatment and educational support and resources.

DEPARTMENT DESCRIPTION OR PROGRAM DESCRIPTION HIRING FOR
I CARE supports youth, up to age 21, who are at risk for or have experienced sex trafficking or other commercial sexual exploitation. The program provides a mental health clinic and a drop-in center.
CONTEXT OF THE POSITION
Provide case management and guidance support to Commercially Sexually Exploited (CSE) Youth and those at risk of exploitation throughout San Diego County through unprecedented patience, creativity and acceptance, as youth develop relational competencies and life skills. Manage participant motivation, removal of barriers, implementing case management, intensive care coordination, individual and family rehabilitative support and other goals in coordination with the youth, family and clinician based upon their treatment plan; facilitate referrals and collaborate with community partners as necessary to ensure appropriate levels of care and continuity of care.
Essential functions include:
- Utilize clinical treatment plans to develop goals, provide support and services based on treatment with lead clinicians. Provide comprehensive case management services and rehabilitative services to include alcohol & drug assessments, case planning, solution seeking, goal setting, family building, skill building, crisis triage, and referral and advocacy. Must serve at least 125 unduplicated clients within clinic per fiscal year per FTE and maintain 30% productivity standard of billable services.
- Facilitate 1-2 groups weekly including and not limited to psycho-educational, skill building, CSEC prevention and artistic expression groups as well as pro-social activities and outings in the community for youth and their caregivers.
- Provide assessment, guidance and life planning support to participants during weekly/daily communication as appropriate based on their goals and identified level of care needed. As well as identify individual/family strengths that can be applied to overcome identified family challenges that prevent/hinder the family from maintaining a safe home environment.
- Provide support in the form of reflective listening, emotional support, strengths identification, challenge identification, and solution seeking as needed.
- Maintain professional and appropriate levels of coordination of care with outside providers of the youth’s treatment team including but not limited to youth’s probation officer (PO) and/or youth’s Child Welfare Services Social Worker when applicable.
- Provide linkages and referrals to community-based organizations and resources as needed to support clients’ needs.
- Conduct community outreach including but not limited to providing CSEC trainings and general outreach to potential referring partners as well as to youth and families directly, which includes developing and establishing collaborative partnerships with community providers and members.
- Ensure that all medical records are maintained according to the agency, program, and County standards for documentation of services including but not limited to behavioral health assessments, service plans, progress notes, medical history, and legal consents.
- Provide onsite coverage and supervision of youth in the milieu at the Drop-In Center during established hours on a rotation with other team members.
- Transport participants when necessary.
- Available to be “on call” in order to provide in-person support at the Drop-in Center or in collaboration with the CSEC Response Team (CRT). “On call” schedule will be determined by program manager.
- All other duties as assigned by Program Manager.
PREFERRED PROFESSIONAL EXPERIENCE AND QUALIFICATIONS
- Bachelor's degree in social work or related field with two years’ equivalent experience.
- Two years’ experience working with at-risk youth preferred. Related experience working with Survivors of Commercial Sexual Exploitation and/or Human Trafficking preferred.
- Experience in case management preferred. Must be willing to develop or possess working knowledge of trauma-informed care and dual diagnosis treatment; experience working in trauma-informed and dual diagnosis programs preferred.
- Demonstrated leadership and problem-solving abilities, training skills, creativity, and public speaking. Ability to provide quality trauma-informed services in a fast-paced environment, specializing in crisis intervention and de-escalation, and motivational interviewing.
- Culturally sensitive and knowledge of resources in San Diego County.
- Ability to (re) prioritize and multi-task is a must.
- Available to work evenings by client requested appointments and some holidays in accordance with the established clinic hours.
- Must be able to pass a clearance for working with minors.
- Valid CA driver’s license, good driving record and must be able to utilize own properly insured vehicle on the job (mileage reimbursed).
- Maintain certification in CPR and First Aid. Employment is subject to required clearances which include tuberculosis test, fingerprinting, background checks, and drug testing.
PHYSICAL REQUIREMENTS
- Must have ability to bend at the waist, kneel or crouch to assist youth. Must have ability to hear and speak to exchange information and make presentations.
- You must have ability to lift and carry 20 pounds--lifting books, materials and equipment to execute groups and presentations.
- Seeing to read, prepare and review a variety of activities and to monitor youth activities.
- Sitting or standing for extended periods of time.
- Walking extended lengths to move around campuses.
- Potential exposure to physical injury from aggressive behavior.

Salary Range: $21-23/hour
