Transitional Case Manager – Bilingual Preferred

San Diego Youth Services seeks a Transitional Case Manager – Bilingual Preferred in the Youth Emergency Shelter (YES) Program to be based at Uptown Youth Center.  This is a full-time & non-exempt position reporting to the YES Program Manager.

We provide safe places to live and long-term solutions through shelter, foster homes, community centers and housing to more than 28,000 youth every year at 100+ community and school locations. Through prevention, early intervention and treatment, we also help youth before they need higher levels of care or become homeless.

  1. Meet the basic needs of youth.  Hunger, poverty and youth homelessness are on the rise for many youth and their families today. We help meet these basic needs for youth. What could be more vital than food and shelter? We provide safe places to live and long-term solutions through shelter, foster homes, community centers and housing.
  2. Create positive connection. We help youth focus on positive life changes and healthy relationships as part of providing needed resources and services. San Diego Youth Services has long provided foster homes and adoption support services for families considering adopting a youth in the foster care system.
  3. Help youth develop life skills. In addition to a range of safe and stable housing options, we offer support to help youth develop the independent living skills necessary to become self-sufficient and keep off the streets.
  4. Promote overall health and well-being. Early intervention and care for the whole child are critical. We provide a broad continuum of services to help youth overcome trauma and challenges that put them at risk, including case management, counseling, mental health treatment and educational support and resources.

DEPARTMENT DESCRIPTION OR PROGRAM DESCRIPTION HIRING FOR
Our Youth Emergency Shelter is the only emergency night shelter in the county for homeless and runaway youth. We provide shelter and support services for children and teens, ages 12 to 17, who don't have a safe place to live or are victims of physical or sexual abuse, neglect or exploitation.

CONTEXT OF THE POSITION
Provide case management, independent living skills assessments, life skills/health education groups and crisis intervention services to aid youth in transitioning from emergency shelter to transitional or permanent housing. Assist youth in developing skills and life plans that will support getting them off the streets and that will support their transition to self-sufficiency.

Essential functions include:
  • Provide trauma-informed care coordination for all case management and ILS related services, including crisis intervention, advocacy, information, referral and other direct services, to aid youth in transitioning from emergency shelter to transitional or permanent housing.
  • Welcome and treat youth with co-occurring mental health and substance abuse disorders as having two primary conditions each requiring attention.
  • Assist youth in accessing options for medical health, mental health, education, and employment.
  • Transport (or arrange transportation for) youth to and from appointments when necessary.
  • Facilitate psycho-educational groups for youth, including life skills and health education groups.
  • Develop and provide service-learning opportunities for youth
  • Engage parent/guardian to support reunification, when possible, and/or transition to self-sufficiency.
  • Maintain linkages with community referral sources.
  • Attending all required meetings.
  • Welcome and support volunteers in the organization and assist them in accomplishing their services whenever applicable.
  • Maintain and ensure maintenance of data collection and contract compliance of records as required.
  • Assist in maintaining clean, warm, welcoming office grounds and facilities.
  • Have availability to work on weekends and evenings.
  • Maintain a working knowledge of MIS equipment.
  • All other duties as assigned by Program Manager.
PREFERRED PROFESSIONAL EXPERIENCE AND QUALIFICATIONS
  • Bachelor’s degree in social work or related field, or equivalent knowledge and experience, and two years relevant experience.
  • Two years’ experience working with high-risk youth preferred. 
  • Must be willing to develop or possess working knowledge of trauma-informed care and dual diagnosis treatment; experience working in trauma and dual diagnosis programs preferred.
  • Able to work in a very diverse setting with people of all backgrounds, culture and orientation. Demonstrated leadership and problem-solving abilities, training skills, creativity, and public speaking.  Bilingual (English/Spanish) preferred. 
  • Available to work some evenings, weekends and holidays.
  • Able to be clear to drive and have a valid driver’s license.
  • Able to obtain and maintain CPR/First Aid Certification.
  • Employment offers will be subject to clearances, which include background checks, TB, and drug testing.

Salary Range:  $20-22/hour
